I hereby grant permission for my child to participate in all activities organized by Learn Haven during the summer camp. In consideration of my child’s participation, I acknowledge that every reasonable effort will be made to ensure a safe and enjoyable experience. However, I understand that certain activities may involve inherent risks.

In the event of injury to my child, I waive all claims against Learn Haven, including its coaches, affiliates, participants, sponsoring agencies, advertisers, and, if applicable, the owners and lessors of the premises where the camp is conducted. I recognize that participation in camp activities may involve risks such as minor cuts or scrapes from crafting or building projects, exposure to materials that could cause mild allergic reactions (e.g., certain paints, glues, or other art supplies), and the handling of equipment like computers, robotics kits, or other technology, which may present minor hazards if not used properly.

As the parent and/or guardian of the named child, I hereby authorize the staff, counselors, and medical professionals associated with Learn Haven to administer necessary medical treatment, including first aid and emergency medical procedures, if my child becomes ill or injured while participating in any Learn Haven programs.

In the event of a medical emergency where I cannot be reached immediately, I grant permission for Learn Haven to seek medical care and, if necessary, transport my child to an emergency medical facility or physician of their choice, as deemed appropriate by program staff.

I understand that I am responsible for any costs associated with my child's medical treatment, including but not limited to transportation, hospitalization, and emergency care. Every effort will be made to contact me as soon as possible in such situations.

Additionally, I authorize Learn Haven, its affiliates, including Directors, Coaches, and Program Staff, to provide necessary emergency care until my child can be admitted to a medical facility.

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
